Card Renewal

You may renew your card before the expiration date in the following ways:

In person at Chivawattana Counters.
Facsimile renewal
- Contact Chivawattana for a renewal form and one will be faxed to you;
- Fill out the form, and indicate your method of payment;
- If money has been transferred to the hospital account, please include your pay-in slip with the form when sending by fax;
- If paying with a credit card, please include credit card details and your signature when sending the renewal form.
For renewal by mail, follow the same steps as fax renewal.

Additional Information

You must renew your card within 1 month from the date of expiration in order to receive the discount.
After the one month period, members are treated as new members and will have a waiting period of 7 days before some benefits can take effect.
